I would like to change insurance companies because i am not really happy with the service I am getting. The thing is, I have a chronic illness.

Can I change to another insurance company even if I am chronically ill?

Any advice would be appreciated

That means you can terminate your current policy at the end of the year and take out a new policy for the coming year.
