Online comparison service moneyland.ch awarded gold, silver, and bronze medals to the cheapest mandatory health insurance offers for 2027. The award process accounts for 152 different offers from all 32 providers of mandatory health insurance. The results show that a handful of offers are consistently cheap across premium regions.
Like every year, the coming days will see residents all over Switzerland receiving mail from their health insurance providers informing them about the premiums for 2027. On average, premiums are going up by five percent. Many residents will be looking for a cheaper offer. The difficulty is that the selection of offers is huge, making an overview difficult.
Insurance companies offer mandatory health insurance with up to seven different deductible models, and multiple insurance models. In addition to standard mandatory health insurance, insurance companies may also offer managed care models like the family doctor or telemedicine model.
Online comparison service moneyland.ch analyzed 152 mandatory health insurance offers from all 32 providers of obligatory health insurance to create a comprehensive analysis. The study shows how the cost of each offer compares to that of the cheapest offer available.
Which offer received the most awards?
Applying the award system used for the olympic games and international sports championships, moneyland.ch awarded gold, sliver, and bronze medals to the cheapest health insurance offers. This award system makes the complex insurance premiums easier to understand. It shows how often a mandatory health insurance offer is the cheapest in one of the 42 premium regions. Separate competitions were held for the lowest deductible (300 francs) and the highest deductible (2500 francs).
The app-based managed care offer Agri Smart from Agrisano is the clear winner, with 24 gold medals. "In other words, that Agrisano offer is the cheapest mandatory health insurance in 24 of the 42 premium regions" clarifies moneyland.ch personal finance expert Ralf Beyeler. Next in line are the Sanitas Telmed Basic offer with 18 gold medals, and KPT Win Smart with 10 gold medals.
The comparison with 2026 reveals some interesting developments: "In the last analysis, Agrisano's Agri Smart offer took 35 gold medals, while Sanitas only received seven. So while Agrisano can still hold its position, Sanitas has made major gains," observes Ralf Beyeler.
If you also account for silver and bronze medals in addition to gold, then there is a different winner. Win Smart from KPT received a total of 35 medals, followed by Agri Smart from Agrisano with 33 medals. Sanitas Telmed Basic and ÖKK Casamed HMO received 29 medals each.
How much does an offer cost compared to competing offers?
To find out, moneyland.ch also conducted a countrywide analysis. This analysis only accounts for offers that are available in at least 28 of the 42 premium regions.
The analysis shows how much a mandatory health insurance offer costs in relation to the cheapest available offer. The lower the average markup is, the better the ranking of that offer compared to other offers.
In this analysis too, Agri Smart from Agrisano is in first place. On average, Agri Smart is just 2.3 percent more expensive than the cheapest available offer. While Agri Smart is the cheapest offer in many premium regions, there are also regions with cheaper offers. If you only account for the regions where it is not the cheapest offer, the average markup is 13.7 percent. It is worth noting that Agri Smart is not available in many large Swiss cities, including Zurich, Geneva, Lausanne, Luzern, and St. Gallen.
The runners up include offers from KPT, Sanitas, ÖKK, and Concordia. On average, they cost between 3.3 and 4.7 percent more than the cheapest available offer. The markups for these offers are as high as 17.3 percent, depending on the premium region. There are other offers with much higher markups. The insurance offer with the highest average markup cost 39.2 percent more than the cheapest available offer, on average. The biggest difference between the cheapest and the most expensive offer in the same premium region is 109.6 percent.

"That the most expensive offer can cost more than twice as much as the cheapest offer for the same location is remarkable. The figures show that comparing insurance offers remains as important as ever," says Ralf Beyeler with regard to the huge differences in premiums.
Which is the cheapest managed care model?
There are substantial premium differences between insurance models. Across all offers as a whole, the app-based managed care model is the cheapest, on average. The average markup compared to the cheapest available offer is 7.5 percent.
Currently, only Agrisano, Aquilana, Atupri, KPT, and SLKK offer the app-based managed care model. Before getting medical care, the customer must first use the app-based symptom checker. The AI-based symptom checker creates a first evaluation of the condition, and either makes a recommendation for self-treatment, or refers the customer to a general practitioner or other service provider. Depending on the offer, the referrals made by the AI-based service for treatments may be binding.
Other insurance models have higher markups, at 12.1 percent for HMO-based offers, 12.2 percent for hybrid model offers, 13.0 percent for family doctor offers, and 14.3 percent for telemedicine offers. Standard mandatory health insurance without managed care costs 28.2 percent more than the cheapest available offers, on average.
There are some insurance providers with pricing that reflects that of the overall market. But there are also some insurers whose pricing deviates sharply from the averages. The graph below shows the markups for different insurance models by health insurance provider.
Managed care models may be much cheaper than standard mandatory health insurance. But in exchange for lower costs, managed care models require you to follow strict rules to benefit from insurance cover. Before getting medical care, you must always consult the specified gatekeeper first. There may also be restrictions on your choice of doctors and hospitals, as well as medicines and pharmacies. You can find detailed information in the guide to using managed care models for Swiss mandatory health insurance.
Methodology
The evaluations were conducted by moneyland.ch using the final premiums for the year 2027 published by the Federal Office of Public Health (FOPH). The analysis accounted for all mandatory health insurance offers, without accident cover, for the 300 franc and 2500 francs deductible models. When an offer is not available in a certain premium region, it is excluded from the analysis for that premium region.
General methodology for all evaluations: For each premium region, the premium for the cheapest available offer is used as the reference cost. moneyland.ch then calculated the average difference between the reference cost and the cost of each offer for that premium region. Additionally, moneyland.ch also calculated rankings for each premium region.
Awards: For each premium region, moneyland.ch listed the cheapest, second-cheapest, and third-cheapest mandatory health insurance offer. The cheapest offer received a gold medal, while the next two in line received silver and bronze respectively. In the case that two or more offers have identical premiums, then all of the offers receive the same medal, as per the standard award system used in sports.
Comparisons with competing offers: This evaluation shows the average difference between the cost of an offer and that of the cheapest available offer, across all premium regions. Only offers that are available to residents of at least 28 (two thirds) of the 42 premium regions are included in this analysis.
Insurance model analysis: Each offer is placed in one of six insurance categories: standard model, family doctor model, HMO model, telemedicine model, hybrid model, or app-based model. moneyland.ch then calculated the average markup over the cheapest available offer for all offers in a category, across all insurance providers.
