Mandatory Health Insurance: Who Has the Cheapest Offer?

Independent online comparison service moneyland.ch analyzed all available mandatory health insurance offers to determine which offer is cheapest across the most premium regions. Gold, silver, and bronze ratings were given to the cheapest offers. The results are surprising.

Many residents have already begun receiving their new health insurance policies for 2026 from their insurance providers. Those looking for an alternative to their current insurance often find it difficult to navigate the mandatory health insurance market. More than 30 insurance companies offer mandatory insurance in various versions, including the standard, family doctor, HMO, and telemedicine models. For each of these offers, you can choose between up to seven different insurance deductibles.

To help bring clarity, independent comparison service moneyland.ch studied 156 different offers from 34 health insurance providers to create a comprehensive analysis. It shows how individual health insurance companies and their offers compare to the cheapest offer. “The results of the analysis are revealing, and the winner is surprising,” says moneyland.ch financial expert Ralf Beyeler.

Which health insurance company has the most medals?

To add a little fun to an otherwise dry subject, moneyland.ch decided to rate health insurance offers in the model of the Olympic games. The first, second, and third cheapest offers in each premium region were awarded gold, silver, and bronze ratings respectively. The total of all medals awarded reveals which mandatory health insurance offers have the lowest premiums on a countrywide basis, for both the 300-francs and 2500-franc deductible models. 

“I had not expected that there would be a clear winner,” says Ralf Beyeler from moneyland.ch. Agri Smart from Agrisano is clearly in first place, with 25 gold medals for the 300-franc deductible, and 10 gold medals for the 2500-franc deductible. “That is also remarkable because Agrisano only offers that product in 30 of the 42 premium regions,” adds Ralf Beyeler. According to the Federal Office of Public Health, Agrisano had 186,684 customers on January 1, 2025. That puts it in twelfth place in terms of size across all Swiss health insurance providers and groups.

Other health insurance offers that scored high on the medals chart include products from Sanitas, ÖKK, Sympany, Assura, and CSS. It is interesting to note that for the 300-franc deductible model, only 13 different offers received at least one gold medal. For the 2500-franc deductible model, there are 24 different offers that are the cheapest in at least one premium region. 

How do the prices compare to competing offers?

Faced with the forest of offers, many residents wonder how the price of their insurance compares with other offers. For that reason, moneyland.ch created a countrywide analysis that shows how the premiums charged for mandatory health insurance offers compare to both the average price and the cheapest available offer. The lower the average markup is, the better the product rating in the price comparison. 

In this analysis too, the Agri Smart offer from Agrisano has the lead. On average, this offer is 1.5 percent more expensive than the cheapest available offer. In the premium regions where it is not the cheapest offer, Agri Smart costs up to 13.4 percent more than the cheapest available offer. However, Agri Smart is only offered in 30 of Switzerland’s 42 premium regions. “It is also worth noting that Agri Smart is not available in many Swiss cities, including Zurich, Geneva, Lausanne, Lucerne, and St. Gallen,” explains Ralf Beyeler.

Next in line are offers from Sanitas, KPT, and Helsana, with average markups of between 5.4 and 6 percent above the cheapest available offer. But depending on the premium region, the markups can reach to around 26 percent. The 20 offers with the smallest average markups over the cheapest available offer are listed in the table below.

“A look at the full list of 115 offers reveals huge price differences between mandatory health insurance providers,” says Ralf Beyeler from moneyland.ch. An offer from one insurance provider can cost just eight percent more than the cheapest offer in one region, but 100 percent more in another region. How do different health insurance models compare in terms of pricing?

Price differences between different health insurance models are also substantial. The cheapest model, based on offers from all insurance providers, is the “mobile app” model. On average, offers based on this model cost 8.2 percent more than the cheapest available offer. The average markup is 10.6 percent for the HMO model, 11.4 percent for hybrid models, 12.8 percent for the family doctor model, 14.6 percent for the telemedicine model, and 27.6 percent for the standard model.  

The pricing of some health insurance providers reflects that of the overall market. But there are also insurance providers whose prices for different insurance models substantially differ from market norms.
