Outpatient insurance Sanitas Vital Smart.

The insurer can terminate the contract at the latest 3 months before the end of the contract or calendar year. 

Amount of premiums varies depending on the age, place of residence, gender of the insured person, cost coverage of alternative medicine and cost coverage of health promotion. 
Discount for early payment.
The last age class change is at age 96.

Alternative medicine: 80% of the costs, maximum CHF 2500 or CHF 5000 per calendar year.
Health promotion: 50% of the costs, maximum CHF 400 or CHF 600 per calendar year.

The higher cover is used to calculate the premium.

Cost contribution to eyeglass lenses, contact lenses and vision aids or eye lasering: 100%, up to CHF 300 within 3 calendar years, including neighboring foreign countries.

80 percent cost contribution, up to a maximum of CHF 1000 per calendar year including preventive measures.

Cost contribution of 80 percent for check-ups, gynecological preventive examinations and early detection of cancer up to CHF 1000 per calendar year including vaccinations.

You can choose between two types of insurance cover:

Option 1: Cost contribution of 50 percent to fitness subscriptions and exercise classes, up to a maximum of CHF 200 per calendar year. Cost contribution of 50 percent to other health promotion (nutrition, mental balance, etc.), up to a maximum of CHF 200 per calendar year.
Option 2: Cost contribution of 50 percent to fitness subscriptions and exercise classes, up to a maximum of CHF 300 per calendar year. Cost contribution of 50 percent to other health promotion (nutrition, mental balance, etc.), up to a maximum of CHF 300 per calendar year.

90 percent of medicines not coverd by health insurance companies.

You can choose between two types of insurance cover:

Option 1: 80 percent up to CHF 2500 per calendar year (massages up to CHF 250 per calendar year).
Option 2: 80 percent up to CHF 5000 per calendar year (massages up to CHF 500 per calendar year).

Cost contribution to nonphysician psychotherapy: 80 percent up to CHF 1000 per calendar year together with ligation and correction of protruding ears.

Full cover for emergency transportation and rescue costs. Search costs: CHF 50,000 per event.

Costs are covered if organized by Sanitas Assistance.

Full coverage of outpatient and inpatient emergency treatment for a maximum of 180 days.

Treatment of tooth misalignment up to the age of 20 (end of calendar year): 50 percent (per calendar year), including neighboring foreign countries.

90 percent up to CHF 1000 per calendar year (amount in excess of basic insurance benefit and others according to list).

Cost contribution to ligation: 80 percent up to CHF 1000 per calendar year together with non-physician psychotherapy and correction of protruding ears.

For example antenatal classes, postnatal exercise classes and first aid courses: 80 percent up to CHF 1000 per calendar year. Waiting period: 9 months.

Cost contribution to correction of protruding ears: 80 percent up to CHF 1000 per calendar year together with non-physician psychotherapy and ligation.

Coverage within hospital insurance.

Full cover even with pre-existing conditions (for a surcharge or with restriction).
Increase (with health check) or downgrading (without health check) of the insurance level possible (per first day of the month).
